Church Facility Usage and Hold Harmless Agreement/Disclaimer

 

I/We (Guests of Hidden Treasures Ministries, Parents, Guardians, and Consumers) understand and agree that neither Hidden Treasures Ministries, the Church, nor its trustees, representatives, employees, and agents may be held liable in any way for an occurrence in connections with the Christmas activity which may result in injury, harm, or other damages to the undersigned or members of our organization and guest, invited or not.

 

As part of the consideration for being allowed to use your facility grounds and restrooms, I/We release Hidden Treasures Ministries, the Church, Destiny Community Church, its trustees, employees, agents, or representatives from any claim for damages, injury or death which may or could occur while participating in the Christmas event/activity.  I/We further agree to save and hold harmless Hidden Treasures Ministries, the Church, its trustees, employees, agents, owners of property, or representatives from any claim arising out of or participating in any form or fashion in the Christmas activity.

 

I/We the Participants/Consumer/Guardian, ______________________________ read the above disclaimer and understand If I am injured while participating in this Christmas event I will not hold Hidden Treasures Ministries, the Destiny Community Church, (DCC) (Or owners of the property) nor its trustees, representatives, employees, and agents may be held liable in any way for an occurrence in connections with the Christmas activity which may result in injury, harm, or other damages to the undersigned or members of our organization and guests, invited or not.

 

I/We the undersigned and invited guests invited or not will hold Hidden Treasures Ministries and Destiny Community Church harmless if any injuries or death occur.  Hidden Treasures Ministries shall be using the grounds of 711 N. Parson Avenue, Brandon, FL 33510 (Destiny Community Church) on Saturday, December 6 from 11:00 a.m. to 3:00 p.m. for the purpose of an event, A free Christmas festival for people with disabilities.

 

Names of Participants if more than one.  Each Participant/Guardian must sign before entering the grounds of DCC.
